Coastal Medical patients can be seen evenings, weekends and holidays. If you need an adult sick visit, please call Coastal365 at 800-822-5981 to schedule an appointment.   

Pediatric Sick Appointments 

Coastal Medical pediatricians are available 365 days a year. Please call your child's pediatrician.  

In a true medical emergency, please dial 911 or go to the nearest emergency room.

Request for Medical Records

Requests for patient's medical records will be billed to the patient according to state regulations.

Online Forms for Requesting a copy of Medical Records

• Patient Online Access Form - for individual patients or their legal guardians to request medical records.
• Third Party Request Portal - for healthcare providers, attorneys or those requesting the medical records of someone other than themselves (and who are not the legal guardian).
